The following items are required at the time of your appointment: Current Insurance Card Full Co-Payment Completed Patient Forms (Forms Will Be Sent to You) Any X-Rays Pertaining to Your Appt. If Your Insurance Deductible has NOT been met, you will be  asked to pay at the time of your visit.  IF YOU DO NOT HAVE ALL OF THE ABOVE ITEMS AT THE TIME OF YOUR APPOINMENT YOU MAY NEED TO BE RESCHEDULED.
